Qualifications

​

Applicants must meet the following requirements:

• Current Home Health Aide (HHA) certification or active CNA license

• Ability to complete a Level 2 background screening

• At least 1 Year of hands-on caregiving experience

• Professional License, Caregiver Training Certificate

• CPR Certification

• Driver’s License or State ID

• Social Security Card

• Resume

• TB Test (within the last year) or chest Xray (Within the last 5years)

• Physical Exam or Health Statement

• Required continuing education certificates from an elder affairs-approved CE provider, the agency will provide additional training as needed, and for all Direct Service Providers who choose to work with Medicaid Waiver Consumers
